When mailing your Arizona individual income tax return, include Form 140, page 1 and page 2, along with all required supporting forms, schedules, and withholding statements. If you are filing Form 140-SBI (Small Business Income), include it with your individual income tax return in the same envelope and do not mail it separately.

Mailing Requirements

  • Place all forms and schedules in the order specified in the Form 140 instructions.
  • Attach copies of Forms W-2, W-2G, and any Forms 1099 that show Arizona income tax withholding.
  • Include all required schedules, credit forms, and supporting documentation.
  • Do not staple, paper clip, or otherwise attach your check to the return.
  • If paying by check or money order, make it payable to the Arizona Department of Revenue and write your Social Security Number, tax year, and form number on the payment.

Additional Forms

  • If claiming nonrefundable individual income tax credits, complete and include Arizona Form 301 when required by the credit instructions.
  • Enter your name and Social Security Number on Form 301 exactly as shown on your Arizona income tax return.
  • Include all applicable credit forms, schedules, and supporting documentation required for the credits claimed.

Mailing Addresses

If enclosing a payment, mail your return to:

  • Arizona Department of Revenue
    PO Box 52016
    Phoenix, AZ 85072-2016

If not enclosing a payment, mail your return to:

  • Arizona Department of Revenue
    PO Box 52138
    Phoenix, AZ 85072-2138
