- On Form 540 2EZ, check the “Yes” box on Side 4 if you are interested in no-cost or low-cost health care coverage.
- On Form 540, check the “Yes” box on Side 5 for the same purpose.
- On Form 540NR, check the “Yes” box on Side 5 if you are a nonresident or part-year resident.
By checking the “Yes” box, you authorize the FTB to share limited information from your tax return with Covered California for outreach and enrollment efforts. The shared information is limited and used solely for connecting taxpayers with affordable health insurance options.
