You may need to file Form 8962 if you received a Form 1095-A, depending on your specific circumstances. According to the IRS instructions for Form 8962, you must file this form if any of the following apply: you are claiming the Premium Tax Credit (PTC), Advance Premium Tax Credit (APTC) was paid for you or another individual in your tax family, or APTC was paid for an individual you told the Marketplace would be in your tax family but is not included in any tax family.
Form 8962 is used to calculate your PTC and reconcile it with any APTC that was advanced to your insurance company. If you or a family member enrolled in health insurance through a Health Insurance Marketplace (such as HealthCare.gov or a state-based exchange) for 2025, you should have received Form 1095-A, which details your coverage months and any APTC paid. If APTC was paid on your behalf, or if you wish to claim the PTC, you must file Form 8962 and attach it to your tax return (Form 1040, 1040-SR, or 1040-NR).
When You Do NOT Need to File Form 8962
- If you are claimed as a dependent on someone else’s tax return, that person will file Form 8962 for your coverage, and you do not need to file it yourself.
- If no APTC was paid for you or your family, and you do not wish to claim the PTC, then Form 8962 is not required.
Important Notes About Form 1095-A
- If your Form 1095-A has the VOID box checked, do not use it to complete Form 8962. It was sent in error and should be disregarded.
- If your Form 1095-A has the CORRECTED box checked, use only the corrected form for your calculations and ignore the original.
- For multiple policies in the same state, enter only one SLCSP premium amount (from column B, line 33) on Form 8962, unless married in December and enrolled in separate plans, or enrolled in different states (in which case, total the amounts).
