Ohio Return Mailing Addresses
When filing your Ohio individual income tax return, it is important to send your documents to the correct mailing address. The Ohio Department of Taxation provides specific instructions for mailing your return, especially if you are filing both Ohio state and local (city or school district) tax returns.
General Mailing Instructions
- Do not staple, paper clip, or otherwise attach your return together.
- When filing both Ohio and school district income tax returns, send each return in its own envelope.
- Ensure your return is placed in the proper order and verify that all forms are for the correct tax year.
- Use black ink and UPPERCASE letters for all entries.
- Round all figures to the nearest dollar.
Required Documents for Filing
For city-specific returns, you may need to include additional documents such as:
- W-2s (include a copy to be filed with Employee City or Local Tax Return)
- W-2Gs
- Schedule C - Profit or Loss from Business
- Schedule E - Supplemental Income and Loss (including rental property income)
- Form 1099-NEC/MISC
- First two pages of Federal Form 1040 and Schedule 1
Important Notes
- Do not change the year on the form. If you do, processing may be delayed.
- For city tax returns (e.g., Akron, Cincinnati, Perrysburg, Westerville), follow the specific instructions provided by each city’s finance department.
- Some cities require you to list income by source and combine it by city in the Employment/Profit Tax section.
