Oregon Amended Return
To file an amended return for the state of Oregon, you must complete the return as it should have been filed, including any adjustments made by the Oregon Department of Revenue. You must check the “Amended return” box on the first page of your return.
In the “Amended statement” space, provide the line number of each change and the reason for the change. This helps ensure clarity and accuracy in processing your amended return.

- Do not submit photocopies or use staples when filing.
- Keep all worksheets and supporting documents with your tax records. For example, if you used Worksheet FCG, retain it but do not include it with your filed return.
