This form is required if you or a family member enrolled in health insurance coverage through a Health Insurance Marketplace (also known as an Exchange) for the tax year. The PTC is a tax credit designed to help make health insurance more affordable for eligible individuals and families.

Purpose and Use

Form 8962 is attached to your federal income tax return (Form 1040, 1040-SR, or 1040-NR) to determine the final PTC amount you are entitled to for the year. It reconciles the APTC (which was paid in advance to your insurance company) with the actual PTC you qualify for based on your annual income and household size. You must complete this form only for coverage purchased through a Marketplace, including plans bought via HealthCare.gov or a state-based Marketplace.

  • Ensure all amounts from Form 1095-A are rounded to the nearest whole dollar when entering them on Form 8962.
  • Check your math carefully, especially on lines 11–23, 24, and 25, as errors can lead to incorrect credit amounts or delays in processing your return.
  • If filing as married filing separately and not eligible for certain elections, enter -0- on Line 24.
  • Verify that your Social Security Number (SSN) on Form 8962 matches your tax return. If you used an ITIN, enter that number instead.
