Who is Subject to the Penalty?

Determining Affordability
The affordability of health insurance is determined using the Schedule HC Worksheet for Line 12. This worksheet evaluates whether an individual could have purchased affordable private health insurance in 2025.
- If line 2 (cost of coverage) is less than or equal to line 5 (affordability threshold), the individual is deemed able to afford coverage and is subject to a penalty.
- If line 2 is greater than line 5, the individual is deemed unable to afford coverage and is not subject to a penalty.
How to Report the Penalty
- Complete Schedule HC, Line 12, by filling in the "Yes" oval if you are subject to the penalty.
- Proceed to the Health Care Penalty Worksheet on page HC-11 to calculate the exact penalty amount.
- If you are not subject to the penalty, fill in the "No" oval and skip the remainder of Schedule HC.
Required Documentation
To complete Schedule HC, you must have your completed U.S. Form 1040 and know the cost of enrolling in any employer-offered health insurance plan for 2025. This information can typically be obtained from your employer’s Human Resources Department.
