Ohio State Tax Return Filing Requirements
When filing your Ohio state tax return, it is important to understand the required forms, filing procedures, and supporting documentation. The Ohio Department of Taxation provides specific guidelines for individual taxpayers, including those who reside in cities with local income tax requirements.
Required Forms
- Form IT 1040 – This is the primary Ohio Individual Income Tax Return form. It must be filed for all residents and part-year residents. The form includes two pages and should not be altered (e.g., do not change the year printed on the form).
- City Income Tax Returns – If you reside in a city that imposes a local income tax (e.g., Akron, Cincinnati, Perrysburg, Pickerington, Grove), you must also file the respective city tax form (e.g., Form IR for Akron, Form P for Perrysburg).
- Supporting Schedules – Include all necessary schedules such as Schedule C (Profit or Loss from Business), Schedule E (Supplemental Income and Loss), and Form 1099-NEC/MISC if applicable.
Filing Instructions
- Paper Filers: Use black ink and uppercase letters. Round all figures to the nearest dollar. Do not staple or paper-clip your return. Ensure all forms are for the correct tax year and placed in the proper order.
- Software Filers: Use OLT (Online Taxes) software to add state and city tax forms. Navigate to the "State" section, select Ohio, and choose the appropriate return type (e.g., City Tax Return) to proceed with on-screen prompts.
- Amended Returns: For tax years 2024 and forward, use Line 11 on Form IT 1040 to calculate Amended Ohio Federal Adjusted Gross Income.
Supporting Documentation
- Include copies of W-2s, W-2Gs, Schedule C, Schedule E, and Form 1099-NEC/MISC.
- For city tax returns, provide the first two pages of your federal Form 1040 and Schedule 1.
- Attach any additional schedules if more space is needed for income reporting.
Important Notes
- When filing both Ohio state and school district income tax returns, send each in a separate envelope.
- For credits such as Lump Sum Distribution and Retirement Credit, use the worksheet on page 45 of the IT 1040 booklet and include supporting documentation.
- Verify your name, SSN, and address for accuracy to avoid processing delays.
